Exploring the Case Management Process

This phase is about working directly with the client, including counseling the client and advocating on their behalf.

 

Coordination of Other Services

 

During the coordination stage, a case manager gets an understanding of how other services are working in order to cater to the client needs. This helps the case manager build a working relationship with the client, and thus get a well-rounded view of how to achieve the client’s goals.

 

Monitoring and Review

 

As the case management process nears its completion, it becomes important to reassess everything that has been done to date, see whether the plan has been followed closely or if it needs to be changed according to the client’s evolving needs, and finally if the process has generated the desired results.

 

Case Closure and Follow up

 

The penultimate step of the process is closing the case, following up on any aspects that may be pending, and bringing the client relationship to a fruitful end.

 

Evaluation

 

Finally, feedback is taken from everyone who worked on the case, as well as the client, to determine how effectively the case was concluded, and if the client has been happy with the process and the end result.
