Exploring the Case Management Process

Businesses are always an assemblage of various processes, which we call cases. Every case is a collaborative process that includes screening, assessing, planning, coordinating, monitoring and evaluating the client’s needs to bring it to a successful completion.

The best case management software comprises eight necessary phases. Here’s a look at the various phases in detail:

This is the first step, and it’s a quick one. The aim of the screening process is to make sure that the service a client has been referred to appropriately fits their particular needs. During this phase, the client is informed if the service or the client’s needs are found to be incompatible in any way.

Rather than being just a step in the case management process, assessment is more of a process that continues throughout the case’s lifespan. It’s comprehensive in nature, taking a client’s long-term needs and available resources into account in order to strategize the best way to meet those needs. There’s also crisis assessment, which comes into effect in case of an emergency situation regarding security, safety, health, income and more.

During this step, the client’s needs are identified as precisely as possible, determining actions based on those goals, and strategically prioritizing those actions. Because this part of the process focuses on how to achieve the client goals, it is largely driven by the client.

Direct Service

This phase is about working directly with the client, including counseling the client and advocating on their behalf.

Coordination of Other Services

During the coordination stage, a case manager gets an understanding of how other services are working in order to cater to the client needs. This helps the case manager build a working relationship with the client, and thus get a well-rounded view of how to achieve the client’s goals.

Monitoring and Review

As the case management process nears its completion, it becomes important to reassess everything that has been done to date, see whether the plan has been followed closely or if it needs to be changed according to the client’s evolving needs, and finally if the process has generated the desired results.

Case Closure and Follow up

The penultimate step of the process is closing the case, following up on any aspects that may be pending, and bringing the client relationship to a fruitful end.

Finally, feedback is taken from everyone who worked on the case, as well as the client, to determine how effectively the case was concluded, and if the client has been happy with the process and the end result.
